1. Who we are

Evolve Physiotherapy ("we", "us") is a physiotherapy and Clinical Pilates practice at 35 Selegie Road, #10-07, Parklane Shopping Mall, Singapore 188307. This policy covers personal data collected through this website and when you contact us.

2. What we collect

• Contact form: your name, email address, optional phone number, the topic you choose and your message. Your message may include health information if you choose to share it.
• Online booking: appointments are booked through our clinic management provider (Plato Medical). Our Book page shows Plato Medical's booking form inside this website; details you enter in it go directly to Plato Medical, not to this website, and are handled under their privacy terms and our patient records process. Plato Medical may set its own cookies in that form.
• WhatsApp, phone and email: any details you send us through these channels.
• Technical data: our hosting provider keeps short-lived server logs (such as IP address and pages requested) for security and to keep the site running.

This website does not use advertising or analytics cookies and does not load third-party trackers. The only cookie is a strictly necessary security cookie used when our staff sign in to manage the website.

3. How we use it

We use your personal data only to respond to your enquiry, arrange and provide your care, issue receipts and invoices (including for insurance claims), and meet our legal and regulatory obligations. We do not sell your personal data or use it for marketing without your separate consent.

4. Consent and withdrawal

By submitting the contact form you consent to us using your details for the purposes above. You can withdraw consent at any time by contacting our Data Protection Officer. We will tell you about any likely consequences, for example that we may no longer be able to respond to your enquiry.

5. Who we share it with

We share personal data only with service providers that help us run the practice, such as our website host (servers located in Singapore), our email provider and our clinic management system, and only as needed for them to provide their service. They are required to protect it to a standard comparable to the PDPA. Where data is stored or processed outside Singapore, we take steps to ensure it receives comparable protection. We may also disclose data where required by law.

6. How we protect it

We use reasonable security arrangements, including encrypted connections (HTTPS), encryption of stored data, access limited to authorised staff with two-factor authentication, and regular security updates and backups.

7. How long we keep it

Website enquiries are not stored in the website's database; they are delivered to our clinic mailbox. We keep enquiries only as long as needed to respond and for related business purposes, and delete them when no longer needed (we review our mailbox at least every 12 months). Patient records are kept for the period required by applicable healthcare and legal requirements.

8. Access and correction

You may ask for a copy of the personal data we hold about you, how it has been used or disclosed in the past year, or ask us to correct it. Please contact our Data Protection Officer. We aim to respond within 30 days and may charge a reasonable fee for access requests, which we will tell you about in advance.

9. Data breaches

If a data breach is likely to cause you significant harm, we will notify you and the Personal Data Protection Commission as required by the PDPA.
